Obtain Health Clearance

Once you get your Lincoln email, follow these quick steps:

  1. Log In:
    1. Go to LionsHealth.lincoln.edu by copying and pasting the link into your browser. Students will need to log in with your Lincoln email and password.
  2. Update Your Info:
    1. Enter your date of birth and update any required personal information.
  3. Complete Health Forms:
    1. Click Forms and Requirements from the main menu.
    2. Download the Physical Examination Form and have your doctor complete and sign it.
    3. Upload a scanned or digital copy of the signed form to the portal.
  4. Submit Immunization Info:
    1. Enter the dates for all required vaccines.
    2. Upload a clear copy of your immunization record.

Important: You must complete these steps to avoid a hold on your class registration. Once all required information is submitted, your Health Clearance will be processed. Please allow us 2-3 business days to review and verify this information.

LU Health Insurance Information:
All currently enrolled full-time students are required to enroll. Full-time students are defined as undergraduate students taking nine (9) or more credit hours and graduate students taking three (3) or more credit hours. The applicable premium will be charged to the student’s tuition bill. Closer to the start of the semester, students can choose to complete the waiver to remove the fee.  Click here to view more information regarding the Health Insurance.
